The care delivered includes assessment of the patient’s clinical condition, as well as the symptoms presented and any potential adverse effects associated with treatments. This follow-up allows for continuous monitoring and early identification of relevant changes.
Guidance is also provided on essential self-care measures, including recommendations on nutrition, hydration and appropriate physical activity, in order to promote well-being and recovery. At the same time, patients and their families are educated about warning signs that require closer monitoring or immediate contact with the healthcare team.
Another key focus is support in managing common symptoms such as pain, fatigue, nausea or peripheral neuropathy, ensuring individualised strategies to reduce their impact on quality of life.
Finally, any questions are addressed and close follow-up is ensured, always in coordination with the multidisciplinary team, reinforcing patient safety and confidence throughout the therapeutic process.

In addition, the Grupo HPA Saúde offers a Nursing Follow-up Consultation after chemotherapy, which consists of a telephone contact made by the Day Hospital Nurse within 48 hours following the first treatment cycle. This follow-up aims to assess symptoms and possible side effects, monitor the clinical condition, reinforce adherence to therapy, support medication management, and clarify any questions from the patient and their family.
The main objective of this consultation is to ensure continuity of care, identify complications at an early stage and promote patient safety and well-being immediately after the start of chemotherapy. It is a personalised form of follow-up that strengthens the connection between the patient and the healthcare team, ensuring greater clinical safety, reducing risks associated with treatment and contributing to a better quality of life throughout the oncology pathway.
The Grupo HPA Saúde also provides nursing teleconsultations in various areas, enabling patients to receive specialised follow-up remotely, with comfort and safety. These include post-surgical follow-up, urology, follow-up after the first chemotherapy session, follow-up of specialised examinations, gastroenterology and autoimmune diseases, ensuring that nursing care reaches each patient wherever they may be.
